    Smile Sperm quality

    Hi all,

    Guy in 50s but finally found the one and looking forward to our lives together. But my sperm quality is poor (but GP not very helpful in explaining what figures mean). Can anybody give view on this:


    Motility 17 L
    Concentration 34 * 10^6
    Progression 10 L
    Live sperm 12 L
    Normal morphology 6 % (in earlier test it was 12% but above were lower?).

    Would this mean that even with IVF/ICSI and healthy eggs prospects would be poor?

    Hey Geoff! I’m not much help with the numbers but after lots of IVF failures, and for many reasons, we finally had a positive. The dr said it was hubby’s best sample ever, biggest change was the “committed” increased of supplements he was taking (it was always such a chore to get him to take his vitamins!). Start on high dose antioxidants that should be a good start using point. Not an overnight fix so start taking them a few months before you start your IVF cycle. Good luck! Hope someone can help you with the numbers.

    I'm not an expert on sperm quality (aside from knowing that 4%+ is normal for morphology, so in that respect you're good!).

    I'd absolutely recommend talking to a fertility specialist to understand your results and discuss options - you might even be able to do an initial telehealth consultation if travel is a factor.

    And - right now - start taking Menevit (recommended by our FS) and look at your diet and lifestyle factors (no smoking, no recreational drugs, less alcohol, less caffeine - assuming any of these apply to you).

    It takes ~ 3 months for these changes to show results in sperm production, so start now

    You could also consider having a DNA fragmentation test, if the FS thinks it's worthwhile - costs $, but peace of mind, in my opinion.

    If natural conception isn't likely but all female factors are ok, and you have at least some healthy sperm that the scientists can use for ICSI, you'll get your baby

    I personally know a couple who needed ICSI because he had treatment for leukemia in his 20s. He froze semen back then, but it's interstate, and instead they've been able to create embryos using fresh sperm.

    An FS should interpret the results for you...they will be able to determine the likelihood of getting good sperm.

    We did PICSI (different way to select sperm but still uses ICSI to inject sperm into egg) due to male factor issues.

    Regardless, recommend taking lots of antioxidants and vitamins (a naturopath should be able to assist) and regular clearing of pipes makes sure the dead sperm is out to make better selection on the day of egg retrieval.

    Hi Geoff and welcome.
    My partner has similar issues, with lower numbers than your own. We're about to start round 7.
    You should also ask about DNA fragmentation, always good to know as much as you can. This can affect how embryos develop, mainly from day 3 onward as the male DNA activates.
    Seconding comments above - take all the supplements. I recommend that both you and your partner read the book "It Starts with the Egg" for advice.
    Diet/healthy living etc helps, but is long term and requires min 90 days before it will have an impact.
    Acupuncture is also not only beneficial for females but also males, so if you're comfortable trying this, then it might be worthwhile going to see a specialist fertility acupuncturist.
    Finally, your clinic should talk to you about aspiration over fresh ejaculate. This is done with local anaesthetic in clinic and can achieve improved quality and numbers in samples.
