I used to half fill the bottle with cooled boiled water then take a thermos of boiling hot water. When she was ready for the bottle, I would top it up with the hot water then add the formula from a dispenser as others have said and it was a perfect temp.
If I couldn't do that, I would warm the water only in a microwave then add the formula and give it a good shake.
