I don't know if they still exist but there used to be a company called 1cover. Might be worth having a look.

Sorry, I just got back to this. I had found Columbus as well, I spoke with them on the phone and thoroughly read their policy stuff. With the pregnancy extension package. They cover child birth and care of the neonate and repatriation home once able. You have to have completed your trip by 30 weeks, not have any assisted conception, a singleton pregnancy and doctors clearance.
