I have top PHI cover and it doesn't cost the obs services outside the hospital fees. Out of pocket his initial consult was $220, some of which will be rebatable and his management fees were $5200. I think I get $275 back from Medicare and nothing from PHI.
I'm lucky I didn't have to pay the anaesthetist was he only charged the gap as professional curtesy (I'm a nurse).
Apparently the paediatrist will come through tomorrow and I'll be out of pocket for that. My brother said he paid $700, for his.
Then the 12 and 20 week ultrasounds were approx $300 each ( Medicare gave me less then $100 back, I've only claimed one back so far)

Register for Medicare safetynet op. with the pmf you'll reach that pretty quickly
