My dr referred me for it. You should get a pink piece of paper with information about it on it and that will also have some info you fill out too. I don't know if this is the same for every state. They do a blood test around 10 weeks (as advised on the form) and this cost $100 here. You get this where ever you would get a blood test done. Then you need an ultrasound at about 12 weeks. Your dr will refer you somewhere, probably the same place they have already referred you too if you had an early ultrasound. I think the most important thing they have to get is the neck measurement. I only say that because when they have done mine they say that from what they can see it looks like low risk of down syndrome but of course they say that they need to put it all the information together. The ultrasound didn't cost me anything only the blood test. I hope this helps a little.
