I have a question re spotting before period. It occurs approx 4 days prior to period every cycle. I am wondering if this would be due to low progesterone or possibly a lutal phase defect? I have done 1 icsi round - negative (spotting started 11 days past transfer - using progesterone pesseries ) and 1 natural fet - negative (spotting occured 5 days past transfer). Any info would be appreciated.

Spotting before the period may indicate several problems. Firstly, it may be associated with endometriosis. Have you had a specific history taken for this? Have you had a laparoscopy? Some papers suggest that there is such a condition as a luteal phase defect, and potentially, this may benefit from treatment with additional or extra progesterone support in the luteal phase. On a positive note, those patients I have seen with your symptoms who have either had their endometriosis treated and/or have taken addtional progesterone have all fallen pregnant.
