Default 3 yr old not coping with 11mth old sibling

Our 3 year old boy is not coping with the noise our 11 month old girl makes. He was a very needy baby and has never adapted well to her being around, despite our efforts. We have never forced her upon him and respect his wish to not let her play with his toys and be in his space (within reason), and he must use nice manners and ask gently could mummy take
her away from his game etc. But he often carries on as if she doesn't exist. We acknowledge him when he does something nice for her, but generally don't 'make' him do nice things for her etc so as not to form resentment, but it hasn't seemed to work very well.

He has always been very sensitive to noise, and as a baby would cry at noisy toys, and still does not like a hairdryer or similar. Our DD's voice is extremely loud, even to us. Her squeals of delight are quite torturous!! And her 'talking' noises go right through our son, he recoils and says "please make her stop talking mummy". When he is tired or grumpy he just lashes out at her, and screams at her in a most uncharacteristic and possessed way. He is truly disturbed by her in a huge way. We tell him she is a baby and just learning and maybe we could all help her by using quite voices, but he screams and says "it doesn't work mummy!!!", which is quite true.... It is also effecting him in group situations as he has always been the quiet one and he is now yelling at the other kids and displaying anger. What can we do to help him through this stage?

Have you ever spoken to a doctor about his sensitivity to noise - personally it would be a concern for me at this age.
