I am a high risk pregnancy due to a clotting disorder associated with an auto immune condition. I have been on blood thinning injections every day since before baby conceived.

A PE is definitely a medical emergency, & i would expect your friend would have spent time in hospital - or even still be there - while waiting for medication levels to stabilise.

I had a DVT when i was 23 - clot in the leg - which was so big and carried such high risk of breaking off and causing a PE, there was very real discussion about amputation of my leg as a preventative measure. I spent 2 and a half weeks as an inpatient stabilizing my drug levels before docs were happy to let me go (with both my legs)

Your friend needs to discuss pain relief options with her doctors, & not be afraid to ask for help with her other children.

I had a PE in my second pregnancy at 8 weeks gestation. It was caused by a blood clot that dislodged. My leg had been sore for a few days but I thought it was sore from leg cramps so ignored it. I was on clexane the rest of the pregnancy and my baby was born c-section and was and is perfectly fine. With this third pregnancy (26 weeks) I was on clexane from the bfp.
