    Given you are obviously concerned, my first step would be to take him to a dentist/periodontist. Easy to organise, and as a general rule, not too expensive. They can examine his oral cavity in general, ensure his teeth are all healthy, look for any signs he may have a salivary issue, etc.

    If they give you the all clear, I would go back to his ENT Surgeon. If he has had an adeno-tonsillectomy and grommets, then chronic otitis media and/or tonsillitis is clearly not the cause. However, as someone has already mentioned, chronic sinusitis can cause bad breath, but one usually has symptoms.

    Mouth-breathers can have "pooling" of saliva under their tongues and around their gums, and this can be very smelly. This occurs as with an open mouth there is less normal movement of saliva. Once again, a dentist can give you advice on the best approach in this instance.
