    Thanks @Malak, @bbhope, I know from here on its going to be a tough journey.

    I wanted to go straight for a mTESE because I don't want to put DH through two surgeries (plus twice the cost).. I still have no idea what caused his NOA. I keep begging him to ask my MIL if he had undescended testes but he doesn't want to tell anyone anything yet.
    This is such a hard road, ppl are bugging us for babies and we ourselves love kids and have been planning them since day 1.

    DH has said if it comes to it, he won't deny me children and I should leave him. What the??
    As for donor sperm, I just really really hope it doesn't come to that.

    If any sperm are found in DH, will they be suitable for fertilization ? Or do they have to be of a certain quality ?

    Sorry to welcome you to the club @MrsSIG. I don't get to reply here much as my 27 month old DD, keeps me pretty busy. She is an amazing donor sperm child who I wouldn't swap for anything.

    As with bbhope we did a lot of soul searching and counselling to get this point.

    Unfortunately sperm quality does matter. Way back when (I have been on this journey for over 10 years) DH had a TESA and we found 9 sperm. They were used but did not give good quality embies. Even with donor our embies were not good quality so my eggs were a problem as well.

    Good luck with the journey. Be kind to yourselves and prepare yourself to make very difficult decisions.

    @MrsSIG: did your DH have a surgery when he was a child? If he had, then the surgery is likely for correcting the undescended testes.

    No one ever wants to go down the path of using donor sperm on this thread. That's the last resort. My DH also said the same thing. I could leave him if I want. They are just very depressed about the whole thing.

    Yes quality does matter. They retrieved 5 sperms and only 3 usable. 2 embryos with a very promising blast. Unfortunately, it ended in chemical pregnancy. And the second is less quality and also another chemical pregnancy. My FS left it for us to decide if we wanted to ask the surgeon to perform another mTESE. At the time, the surgeon only looked in one and left the other alone due to poor prognosis. He actually only found one abnormal sperm during the surgery and the others were found later in the lab.

    If your DH has undescended testes, yes, I would recommend to go straight into mTESE. Happy to pass on surgeons names in Sydney. We live in WA and went to seek help there since there isn't any micro surgeon and the donor wait list is long here.

    @MrsSIG my DH said exactly the same. It is just their way of dealing with it.

    My FS said that sperm quality it is the least of your concern because even if you have perfect sperm if the egg quality is crap it won't matter.

    It is also recommended that you get tested to see your egg quality as that may change your choices and time frame. If my egg quality had come back really bad then we probably wouldn't be thinking about a mTESE.

    @bbhope please do let me know of the good microsurgeons here in Sydney. I've heard of dr golovsky, I hope we can get him. Is there anyone else? Unfortunately I have no idea if DH had undescended testes. He thinks he hadn't but won't ask his mum. Grrr

    @Malak, how do you test egg quality ? Is that the Amh test? I just did a blood test last week for it. Won't know the results till next weekend when we have our follow up with the fs..

    @MrsSIG: if you are looking for an urologist. there are a few. DrGolovsky and a recent one, Dr Kim, who is trained under him. Both at IVFA. Some FS also know how to perform mTESE. They just don't do it for living. Someone on this thread went with Dr Pearson at IVFA. With him, you don't need another FS. Basically, he will handle both you and DH. Dr Lok at Ganea is worth looking into. We went with dr golovsky because we wanted an urologist to perform the surgery. I had a FS coordinating with him.

    Yes egg quality is important. I haven't stopped wondering if it was my poor egg quality rather than DH sperm. Who knows. That's the past and we can't turn back the clock. I have my beautiful son in my arms which is more important to focus on his wellbeing.

    Thanks @bbhope, we are currently seeing a different specialist at IVFA, but will asked to be referred to dr golovsky or dr Kim..

    Ah I am very worried about egg quality now. I had an ultrasound yesterday which showed many many follicles on my left ovary. The sonographer was not concerned at all, but it looked textbook pco to me. I have very regular periods , so I never thought I had pcos. I have heard that having pco or pcos can worsen egg quality and make ivf harder :'( .. I'm 27 but i may have eggs that are bad because of pco....one more thing to worry about. Our follow up with the FS is next Saturday (21st), seems ages away and all I can do is google things.

    Oh, and DH doesn't have any scars I don't think, so it must be something else. He didn't have mumps. He did have encephalitis as a child but that's not a risk factor.... This is so confusing trying to figure out what couldn have cause the NOA
