    Done but I have to say the survey seemed confused. I'm not sure exactly what data it's trying to find but it seemed a bit superficial. Is it about violence? FV? Gambling? Relationships? alcohol ?

    LOL. I love how a bunch of forum users are trying to tell Doctors how to create a survey. I doubt they're confused or need any assistance in getting the data they're after
