    Just details of birth, signed off by a doctor or midwife I think it has place of birth, hospital, drs name and the provider number maybe? You give that to centerlink to claim baby bonus or PPL

    If you decide to change her name down the track, baby will always have to fill out the part where it says that they have a previous surname!

    That's ok it's only my name he will still be on the birth certificate

    When we had DD. The hospital filled out her details (date/time of birth/weight & nationality I think. My details & doctor signs & stamps - to prove their was a real baby etc)

    We filled out the rest. I think it's a good idea to give baby your name until married. I wouldn't want a separate name from my baby x

    To change babies surname later on you will need to fill in a name change form from BDM and lodge it & pay the fee
