    Default Birth certificate

    Hey

    Whats the process with the birth certificate in the hospital .
    Who fills it out ?
    Will the hospital write your names on it (farther & mother ) or is that only for you to fill out ?

    The hospital has only given me the forms never filled them out I've had to all 3 times

    The hospital only records the babies details & mothers name etc and its up to you to fill out the rest

    Then do you take them and send them off or does the hospital ? Do they sign it once it's filled out ?

    This....you get a new baby pack with centrelink forms and birth register forms which you fill out and send off.

    You take the form home and post it to BDM later on.
    You can put the father if you want, you don't have to.

    They'll give you the form (it's a government document) that you have to fill out with all the details, your details, dad's details (if applicable), babies details etc then send off to births, deaths and marriages. It you pay you can also get a copy of the certificate, I think it's $44?

    Hospital doesn't full out anything for registering the birth. Only the proof of birth document that you need for Centrelink purposes

    Ok thank you !
    Is it hard to change your baby's last name once your married ?
    My partner has agreed to have her last name as mine until he proposes and we are married ... ( not that I'm worried )
