I am the same, I do ok falling asleep (I read until I can't keep my eyes open!!) but I wake at 2 or 3 to pee and take 1-3 (sometimes 4) hours to fall back asleep, so frustrating!
My therapist gave me a few apps to use when this happens (yes apps!). But they do work as long as I don't get too used to them. So I have to mix it up a lot! One is a breathing exercise and another is a sleeping meditation, I also have an insomnia hypnosis mp3 that does really work but I've gotten to used to his voice and now he annoys me! My therapist is also having me learn mindfulness, which is pretty much teaching your mind to think about the present and not worry about the past or future. Hopefully soon it will all start to work!
