Default high dose gonal f and luveris

    Hello ladies

I will be starting my 4 th round of IVF. We've decided to try 600 iu of gonal f cd 2 and then adding luveris, this will be the first time I will be doing a high dose, normally it is 450 gonal f so I am pretty nervous! i have unexplained infertility and a low amh of 5 and 8 eggs are the most I get, 1 x transfer and no frosties. Thats the reason why we trying a higher dosage this the time and FS recommended luveris to improve egg quality. Last 3 rounds were all BFN. He also mentioned saizen but didnt push it as it seems 'its neither here nor there' in terms of it actually working- there seems to be not enough proof that it does actually work while there are some who swear by it.

Have any of you ladies tried such a high dose of gonal f or luveris and did it do the trick for you? Iam also thinking of lipiodol flushing - anything to give it my best shot for this cycle! Anyone in a similar situation out there? Iam worried such high dose may fry my eggs and they're not that great to begin with!

    Ann

Luveris was one of the things in my successful cycle. I think it was an essential part of that success. Even though we still only had morulas on day 5, this time all the fertilised eggs were day5 morulas with no losses on the way, and both embies that we transferred implanted.
I averaged 8 mature eggs per cycle. It's a good number.
    Good luck.

    Hi Felicita

Thanks for your reply. Congratulations on precious twins! I hope you dont mind me asking how many cycles did you have and if you used gonal f? was there aything else significantly different in your bfp cycle? FS does say that luveris is for better egg quality, I hear its expensive as well but FS said he will give it to me for free so iam pretty lucky I guess! Iam thinking if i up my dosage i may get more eggs although i dont think thst it really works thst way!

    Hi ann
We did 8 egg collections (averaging 8 mature eggs each time, range 3-13), had sperm to do ICSI 7 times, had 7 transfers using 11 embryos in total. 0 FETs.
Most of my cycles were GonalF, but my last was Puregon. I don't think that was a big difference.

In my last 3 cycles I had LH supplementation. 3rd last was Pregnyl when LH was spotted a bit too low. 2nd last was Pregnyl every 3 days from the get go without waiting for LH to drop. Last was Luveris, but I don't remember what day that started on. The only time we got a d5 blastocyst (only an early blast, mind you) was the 2nd last cycle.

    Other things we did:
DHEA, but was on that for 4/8 stims (#4-6, #8).
Donor sperm, but had already used that donor for 2 cycles, and a different donor for the previous 5 batches of eggs (4 fresh batches, 1 frozen).
Assisted hatching. I think this was a big help. Our growth was always slowing by d3 and dropping off a lot thereafter, but picked up after the "eggshells" were thinned. We also did assisted hatching on the previous cycle. Perhaps my eggshells are less permeable than they should be? and would have benefited from sperm bombardment of natural conception or IVF instead of ICSI?
I managed to get the acupuncture right before and after transfer (one of the benefits of Maroubra). Previously I'd only managed some sessions during the stim and TWW.
Switched from crinone to pessaries, but that was just because we were changing everything for the fun of it. Crinone's better since it's slow release.
I travelled across Sydney for treatment. My FS at Westmead referred me to a colleague at Kogarah/Maroubra for the final cycle (part of changing everything for the fun of it). The train trips between Blaxland to Kogarah forced me to rest. The walks at Kogarah and Sydney Central station forced me to do some light exercise. Changing embryo labs was also part of changing things for the fun of it.
Prednisolone and clexane, but had been on that for other cycles too. My NK results were equivocal so FS figured what the hey, if I can tolerate the side effects it can't hurt (figuratively speaking).
Ditched the fancy TTC vitamins and just took folate for the last cycle.

Prior to final cycle did Light'n'Easy just so I'd be reminded to eat my fruit (the weak point in my diet). Also did AIM vege powders, some probiotic capsules and fibre prior to cycle.

With the first donor we did some digital high-mag, but it didn't help us (fert rates dropped, embryo quality was no better).
Partway through the journey we did PolScope on my eggs to check quality of spindle, but the results were appropriate for my age so didn't bother with that again.

We transferred 1 for my first transfer, for the cycle when we only had one growing on day 3, and for the cycle when we had the early blast. We transferred 2 every other cycle. We never had anything to freeze - even the ones we transferred were well below par for freezing.

Hi Ann, I used high dose Gonal F (600) on my last cycle (#2) and have had Luveris on all 3 cycles. My most recent cycle (#3) we used 450 Gonal F and then upped to 525 for the last few days, also had saizen from D2-2 days before EPU.

As you can see from my signature 3rd cycle is the first time we have had transfer, cycle 2 we had 1 morula on D5 and FS suggested holding off till day 6 to see if it became a Blastocyst but it had arrested at a morula and so was cancelled.

I also have a low AMH and have also been on DHEA, melatonin, macca root capsules, royal jelly capsules, co q10 plus, ultra greens, zinc, vit E,D and C, folic acid, berocca, aspirin, prednisone and nitrodur patches (most of these at FS suggestion, some at my own).

This cycle FS wanted to use lower Gonal f dose to give ovaries a bit of a break and with addition of saizen seems to be the most positive result so far, still remains to be seen how it goes. I wish you lots of luck.

Default high dose gonal f and luveris

I tried 900 gonal. Yes you read that right. Made no difference to 180 to be honest. Good luck though.

    Hello ladies

Thank you all for your replies.

Felicita - wow 7 ICSI's ! Must have taken its toll on you.....but you did succeed in the end with the most precious gifts from heaven! I also had acupuncture for cycle 2 and 3 but only on the day of ET.

    IVF 1 - 2 eggs - 2 fertilised 3dt BFN
    IVF 2 - 7 eggs 1 fertilised!!!! I was shocked 3dt BFN
    IVF 3 8 eggs (6 mature) 5 fertilised 5dt BFN

I strongly believe that IVF 2 the low fertilisation rate was due to using frozen sperm as my husband was away in overseas due to work - so it wasnt much fun doing the cycle alone. Also I changed FS for the 3rd cycle, my first FS wasn't great and changed clinic. Iam with QFG now and thats the best thing I did. I trust my FS infact he comes highly recommended and my last cycle was the best cycle although BFN.

Prettypetal -I used Gonalf 450 from cd 2 in my last cycle as well. Infact FS wanted to put me on 450 again as I have responeded to it previously and did have 8 eggs (although only 6 mature). But it was my idea to up the dosage - he went along with it but did mention it wasn't going to make much of a difference but if it made me happy.....however, I am now re-thinking that strategy! Mainly because 450's a tried and tested method - yes while I didnt get my BFP however, I always did make it to transfers. My first 2 cycles used a lower dosage. So Iam rethinking because I dont want to jeopardise my chance in this cycle. I am taking DHEA (since 2011), Royal Jelly and Elevit. I tested my AMH in 2011 but will be retesting it again soon - be interesting to see the numbers! When is your Beta? I wish you lots and lots of , please keep us posted. My hubby said give Saizen ago, we are already spending so much money on the IVF, so we wouldnt know if we dont try but Iam thinking maybe I should just give Luveris a go but seeing you use it for 3 cycles is disheartening.

Lilipilly - not sure how it really works but do you think that the dosage didnt work for you because you had a low AMH - what did your FS think? You are blessed to have your sister.

I will be starting my cycle next week. I am thinking of anything and everything for this cycle. I thought I would give PICSI ago as well - however, FS is not really for it - as he explained to me that if sperm is an issue then ICSI is the best way to go as you dont have much of a chance other way but in my case if sperms good then stick to natural, he wants me to speak to the embryologist and be sure as with ICSI there are chances of éggs blowing' up during the fertilisation process!! Another thing to worry about! I thought by doing ICSI I was giving every egg a better chance!

Hey you ladies reading out there, please feel free to jump in this thread and give your opinions, stories negative or positive, 2ww, sniffers, jabbers, UTDers, there are lots of us ladies wanting reading your experiences. As I said I am willing to do anything and everything this cycle - try out things I never have before (just like you Felicita! you are right if you can tolerate it why not! wouldn't know which concoction will get you over the line until you try).

Has anyone had or with Dr Salisbury at QFG?

    Ann

Default high dose gonal f and luveris

Yes you are right, it was my low AMH that is the reason I got no eggs. I wish you all the very best for your next cycle. Please update us.

Hi, thanks for baby dust, blood test is on Wednesday next week, been feeling ok, had a few twinges in my uterus that I am hoping are positive signs. Saizen was about an extra $3000, we used the DHEA from Cycle #2 and then added the saizen this time. If it works it worth the money. We did PICSI with cycle #1 but not with the last 2 cycles, FS said we didn't need to.

Best of luck with your cycle

    Hi Prettypetal

I am so sorry for the later reply! Thanks for your post. Gosh i thought that saizen would be $1000! So how have you been going? Not too far from you BT now, have you sneaked in a HPT? What is your AMH level?

    Ann
