Default help much appreciated for school project :)

Hello I am a year 12 student completing an IRP on Polycystic Ovary Syndrome
I would love approximately 20 people to complete my survey that asks questions about the feelings and symptoms women have with PCOS. I have already posted one once before but the link did not work, and couldnt get ther results! It will take approximately 3 - 5 minutes to complete the questionnaire.
Your participation in this study is completely voluntary. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point. It is very important for me to learn your opinions.
Your survey responses will be strictly confidential and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential. If you have questions at any time about the survey or the procedures, you may contact me at brooke.jones35@gmail.com

Thank you very much for your time and support. The link is below!

    Done.

Although I found with the symptoms you could click on multiple options, as women have more the one symptom.

Done as well and I agree with what has been said above. A top 3 symptoms would have been better.

    Good Luck.
