yerp its an assesment of what you need to do extra for a child every day like
Administer medication, change a peg tube, suction trachey, hook up feeds, test blood, wheelchair transfer, moniter o2 and HR, how many sezirues they have a day why you cant work when there at school etc

it also has a section for thee diagnosis and a section for the dr to fill out as well

you need to qualify for career allowance to get faschia funding career payment is differnt assesment higher needs,

DD qualifies for me to get career payment because of her high needs but we dont get it cause of DHs income, its easy to get career alowance any kid with a mild disability gets it.

What is your little ones condition ?
