    Default Cancelled IVF cycle

    Hi all,

    I'm new to this whole forum thing, have been reading a lot of them over the past few weeks, but hadn't actually joined any or posted, but today was a tough one and have finally realised that I'm going to need more support than I realised.

    I recently started my first round of IVF (I'm 30 and have bad endo and PCOS), DH is 29 and has perfect swimmers.

    Due to my PCOS and the result of my AMH test (29.8) my fs decided to put me on Puregon 125 per day (I was initially going to have the Elonva 7 day injection, but FS was worried about OHSS). I started that on CD3 (monday), had first BT on Saturday. Got a phone call that afternoon to say all looked good and to start the orgalutran on Sunday at 7pm. Did that, then went in for my scan first thing this morning... whilst being scanned (by a tech, not my FS) they thought that I had some cysts (endometriomas, which we knew about) and about 10 follicles measuring about 0.9-1.1 (I think).

    We walked over to to the IVF office straight after with our piece of paper with all the ?follicle/?cyst sizes on it, and when we saw the nurse, she said that based on the size of my lining (1cm) she believed the ?cysts were actually larger follicles and if that was the case I was likely to only get 3 eggs from the OPU and that we should maybe consider cancelling if that was the case. I was pretty upset, and she suggested coming back in later in the afternoon to have a scan done by the FS and then see her straight after for more meds etc.

    I received a call from her about 3 hours later saying that the FS had reviewed the info and decided to cancel the cycle. SHATTERED. I was too upset to ask many questions, but apparently he believed that we would be able to do a lot better next cycle by upping the meds, and that for possibly 3 eggs it wasn't worth going through with it. Shouldn't that be our decision?? Financially it would be a bit silly if we could get a better result, but now I'm left wondering (over my first glass of wine in ages) if one of those follicles could have been "THE" egg. Also wondering if anyone can give me any info on when/if I'm likely to ovulate without a trigger shot, so that we can at least make sure we give it a good go naturally!!! (Even though we know the chances are minimal).

    Any thoughts would be appreciated!

    Sorry for the situation you have been in. Must have been a terrible blow. First cycle is stressful enough without it being cancelled from under you.
    I think based on several factors your FS made a good call. Not all follicles will have eggs (they MIGHT, but might not too). Not all eggs are mature enough to use, so even if you had 3 they might not all be viable to use. Typical fertilisation rate is 70% so by that number even if all 3 follicles had eggs, and all eggs were mature you would be extremely lucky to even have all three continue to day 3. From day 3 to day 5 generally some drop off. Chances are you would be lucky to get 1 from the 3 to day 5. If you started with less than 3 eggs then chance is even lower. Sometimes it take a bit of a play around with levels to work out what works for you, and while very disappointing I think next cycle will see much better results.
    Perhaps ask can you go back to back?

    Another reflection- at the clinic I attend it is more typical to monitor the cycle more closely with US/BT every 2-3 days. This would have meant a poor response would be picked up earlier and FSH levels would be increased during the cycle.
    If I were you, I wouldn't be too happy with monitoring only Mon then next on a Saturday. For exampe: this Stim cycle I have had about 6 BT and 4 US and haven't triggered yet.

    Hope next cycle is better. In the mean time, make a list of questions and go see you FS for more personal answers to what happened I suggest.

    Hi impatient , a cancelled cycle truly sucks- I had one recently too. Although you are feeling shattered right now I have to agree with Fortitude that perhaps its for the best. I noticed that your lining was only 1cm- clinics like anything over 7cm so it may not have been the ideal endometrial environment anyway . Good luck with your FS consult and hopefully they will offer some answers for you

    Hi impatient. I know exactly what ur going thru. I just had my cycle cancelled too. Only had 4 follies even after being on 325 of gonal for a week. Even tho it didn't feel it, its actually good the fs has given us the chance to get the most for our money. We only got 4 eggs in our 1 st cycle. As a rookie i didn't realise that was actually quite low at the time. Anyway we got a pfp out of those 4 eggs so it was really hard to cancel on the last 4 this time. You never know right?

    We've tried naturally this time, who knows if we got the timing right. But this gig is all a about miracles right? !

    It really is a numbers game . But u gotta b in it to win it! You really should get blood tests and scans every 2 or 3 days tho.... how else can they track follies and alter ur doses?

    Thanks for all your responses - sorry for the delay in replying, have been a bit fragile all week so have tried to stay away from the whole IVF thing altogether - although as I'm sure you all know, it has never left the back (middle!?!?) of my mind.

    Fortitude, I think my biggest problem was maybe the way it was delivered - having the appointment with the FN in the morning and thinking that I had a difficult choice to make, and looking forward to seeing FS in the afternoon to hear his thought and ask some questions, then receiving a phone call a couple of hours later telling me that it was cancelled. I think it made me feel like I have absolutely no control, which doesn't sit well with me!! I just wish they had of kept my appointment with the FS and at least told me then so I could ask the questions I needed to!! As for the monitoring situation - the standard at my clinic for an antagonist cycle is starting injections on CD3, BT on CD8, then U/S on CD9. I assumed that was pretty normal, but based on your and musicalgals post it seems a bit strange now. Will definitely question that! As for going back to back - they said in the phone call that I needed to make an appointment with the FN in the next few weeks so we could start with my next AF. Haven't been ready to do that this week, but will hopefully feel less fragile next week. Also have no idea what the injections have done with my cycle, so not sure when to expect my next AF, which sucks cos we were planning on going to NZ in September, but DH is happy to put that off for a while so that we can have another go thankfully.

    Lillac, I'm a bit confused about the uterine lining measurements - I saw 1cm during the U/S and questioned it because I thought it was thin too, but the tech said that was actually pretty good - and that was part of the clinics decision to cancel the cycle, they said that because my lining was already so thick that my eggs were probably about to mature... I think they like it 7mm+ not cm - is that different from yours? All this is so new and confusing!!

    Musicalgal, thanks for your kind words, and so sorry to hear that you had a cancelled cycle also - I guess sometimes we just have to trust that they have a much better idea of what's going on than we do - although I find that pretty difficult being such a control freak!

    Hey impatient. My AF arrived 4 days early after the injections and cancellation! I was expecting it to b later . Who knows eh? !

    I too am planning a trip in Sep. I've put off 3 trips in the past year due to ivf (and I'm a travel agent!) So this time I've thought screw it I'm going to go ahead with it no matter what. Doesn't do me any favours when i don't anyway. Seems like my whole life is spent counting the days in my diary to see what fits in with what as FS suggests no travel during cycles or 1 st trimester.

    I would be stoked with 4 days early!! AF is technically due on 31st August, so will be interesting to see what happens. It was late for my last cycle which I put down to thinking about it too much!! Still need to make an appointment to go in and pick up my next lot of injections, but keep putting it off. Will have to do it this week though just in case she arrives early!

    The whole ivf/life balance thing seems pretty hard! We need to rebook our September trip but it's hard with work and uni and the whole not knowing if the next one will work makes it even harder. It must suck having so many opportunities as a travel agent and being tied down to this whole process! Are you trying for your first too?

    Hey impatient,

    Yes, this will be our 1st. We had a BFP in Feb this year but m/c at 9wks. Once you've had a taste of BFP it's hard NOT to go back for more rounds. Every cycle I keep thinking we've done it before so it could happen again. But then we have a dissapointment, didnt take, or cancelled cycle. aaaargh!!! Last week I was over it. Over everything, IVF, putting life on hold, people getting pregnant when they sneeze, putting on weight, feeling it was all futile and no chance to get stuck in and lose the weight cos of cycling. Not only that, but I booked 3 emergency trips to America in one day for different families with cancer. In one day!! I actually lost my mum to cancer, so it brought it all back, at really bad time.

    But I'm sure I'll get through it. We always do us tough IVFers. I'm still going on my holiday in Sep. FS is making me wait a month anyway. Am hoping I can fit the Day 21 scan in before I go. I'm only away for 7 days. We'll also try for a natural miracle (again) before I go. Ho Hum.

    Hey musicalgal, sorry to hear about your miscarriage - at 9 weeks that must have been devastating :-(. Sounds like you've had a pretty tough time recently, so a holiday is probably just what you need! Where are you off to?

    All of the women I've read about on these forums just blow me away with how strong they are, especially the ones who just keep going year after year, it's so inspirational and really puts my one cancelled cycle in to perspective!!
