My daughter has a special needs trike with harnesses and such to help her when she tires (low tone) but she wants to learn to ride a 'real' bike. DD is almost 6, 100ishcm, and will need training wheels. What size bike would you suggest?

I'd take her with you to a proper bike shop and have a look and sit her on a few. The staff will be very helpful too. The bikes in a bike shop are not that much more expensive than those in the big chain stores (kmart/etc) and you get proper service and help if there are issues after you buy it, so it's worth it.

DD1 has a 40cm bike - that size is apparently suitable for 4-7yr olds according to the instruction manual

Knowing the size difference between Jacob and Alex, I'd get Alex a 30cm as jake is on the 40cm... But I do agree with pp, take her to a bike shop and let her sit on it before you but it as they are all a bit different even if they have the same wheel size.
