Default Non viable IVF pregnancy - heartbroken!!!!

    Hi ladies,

I am really in need of some support and any advice possible.......

I am 25 years of age, recently underwent my first cycle of IVF. Reason why we chose to go down the lane of IVF was due to 2 miscarriages, one ectopic resulting in emergency surgery to remove my right fallopian tube in the space of 12 months.

We were extremely lucky to have 2 embryos reach blastocyst, with the embryo implanted in the hatching stage. Our luck seemed to continue with our first cycle being successful. I was due for my 7.5 week fetal heart beat scan yesterday (meant to be 8 weeks as of today), but unfortunately they only found a sac with no fetal pole. The sac only measured 6 weeks and 3 days, the nurses are under the impression that the embryo stopped growing at about 6 weeks....a non viable pregnancy.

For the past 2 weeks i have had all the symptoms of a healthy pregnancy - morning sickness, exhaustion and sore breasts etc. I am really confused how it is possible that i am still experiencing all of these symptoms if there is no "baby". My Beta HCG and progesterone levels have been progressing accordingly, on paper this was the perfect pregnancy, however, this brings me to my next dilema. Given my history with miscarraiges i had remained on prog pessaries. In between 5-7 weeks i received BT results of my prog at 135, they were happy with the results and advised me to go to one a day, the following week my prog dropped to 90, which caused some alarm. Of course they increased my dosage to two a day immediately. Now i realise that levels of prog at 90 is still high considering in normal conception they only look for levels of 30, but is it possible this was a contributing factor??? I dont want to get into the blame game, as this process is not guaranteed but it is extremely coincidental that the week the embryo potentially stopped growing was the week my prog dropped.

I have an appointment today to see my specialist to discuss the final results and our options.

If there is anyone that has experienced something similar, or has some general advice it would be most appreciate.

    Many thanks in advance. D

    Hi Dbz

I have had this happen to me twice. Oce was a clomid pregnancy and the other and FET cycle.

It is known as a blighted ovum or missed misscarriage. My first one the baby died not long after my 6 week check up. I was totally oblivious to the passing of my baby and got to 10 weeks with out knowing. It wasn't until I had my 10 week OB appointment and had a routine scan in her rooms that my world came crashing down.

There was nothing there just an empty sac. Absolutely devastationg. I had a D&C the very next day.

My next one was only recently. I had and FET in December and we got nice strong HCG levels indicating a strong pregnancy. However I knew something was wrong and others were playing down my concerns. I was waiting anxiously for my viability scan at 7 weeks.

As soon as my FS did his scan I knew right away I had another one. Another kick in the guts so to speak. Just an empty sac. Another blighted ovum. Again I chose a D&C the very next day.

The reson you have pregnancy symptoms is your body hasn't realised that the baby has passed. The sac keeps growing giving off HCG so tricking your body into thinking your still pregnant. It's a horrible thing to do. Natures very nasty trick.

I don't think your progesterone had anything to do with it (I am in now way a DR). Because it was still very early in the pregnancy your progesterone wasn't all that high so it would have fallen very quickly with a non viable (god I hate that) pregnancy. Your pessaries would have kept your symptoms at bay. I had the same thing.

My progesterone was low so we upped the pessaries which saw an increase but not much (my alarm bells were ringing but I was ignorning them). Once I stopped all my supporting meds I started spotting.

It is my understanding that my supporting meds held off my M/C.

I know how hard this is for you. And it sucks. No one should have to go through this. IVF is hard enough.

After my first B/O i went on and did my first IVF cycle and I now have a DD2 who will be 1 next month. So there is no reason you can't have a successful pregnancy next time.

I'm happy to answer any questions you may have good luck.

    Default Hugs to you xoxo

Hi sweety I has the exact same thing happen in november last yr. My signs were all good too sick etc same as you. For some reason I knew something was wrong and the night before my scan I dreamt that would be no heartbeat and that's what happened. The first scan showed b/o but the follow up one showed a sac and fetal pole of a 6wk baby I should have been 8wk. I had a d&c a few days later. I agree with pp its a cruel trick by nature ivf is too hard without that too!!Thinking of you I know exactly as you are feeling right now. Xoxoxo

    Hi Bdz,
My heart goes out to you. I too had the same experience last November. We had the 8 week scan booked in on our wedding anniversary and all the symptoms were there and levels high. That shattering feeling when they explain to you there is nothing there, just the sac was the lowest point so far on my IVF cycle. I could see by husbands heart break. I had a D&C a week later (I wanted it the next day and they couldn't get me in. So I was suffering morning sickness for another week for no reason). Don't beat yourself up. It is just the ****tiest speed bump in this process of IVF. The D&C was easy (until they put me in the pre op room next to a woman going in for her c section (lame timing). I just wanted it over so I could start to heal and move on. don't over analyze it. It just is. It happens and you don't have to be on IVF. look after yourself and your DP.
I am so, so sorry you have to go this. Stay strong. Us IVFers go through so much.

    Dbz,
I'm so sorry you are in this situation. I agree with Lillyponds your situation does sound very much like a blighted ovum with no fetal pole or sac present and they are thought to be caused by an abnormality in the embryo so a lower dose of progesterone would likely have had little impact. In fact a low progesterone (P4) level can be indicative of a problem with the embryo.

I have also had 2 B/O pregnancies, 1 was textbook perfect pregnancy on paper, high hcg and high progesterone but the 2nd one (natural conception) had low P4 from the start. It is absolutely heartbreaking to go to the scan full of hope and to have it so cruelly taken away from you.

One other thing I would add when you meet with your specialist is to consider taking misoprostol (medical management of the miscarriage) rather than only considering a surgical intervention as there can be future risks to your fertility by having a D&C.

    Take care xx

Many thanks to everyone for sharing your experiences and kind words.

I am having another scan on Tuesday to confirm the non viable pregnancy. My Fs would like to give my body a chance to miscarry naturally within the next ten days from Tuesday. If there is no success they will proceed with a D&C, @ starf1sh, the risks of a D&C did cross my mind.

Once the current pregnancy is sorted, I will also be undergoing a whole range of BT's to rule out any gene mutations etc. Still a long road ahead.

Once again thank you so much for sharing your experiences, best wishes to all.

    Dannielle
