    Default Homozygous MTHFR

    Hello

    Hope you can help?

I have unfortunately experienced 2 missed miscarriages this year, one at 11 weeks, bub got to 9.5w and the second at 9.5 weeks, bub 7.5w. Both times no idea until the ultrasounds.

We have been fortunate to have extensive testing performed and found out that we are both homozygous MTHFR, I am at position C677T and my partner is at A1298C. We also found out second bub had trisomy 22 so miscarriage wasn't related to the MTHFR. All other tests came out fine.

My question is would you put a patient like myself on Clexane or just aspirin during my next pregnancy? I am taking aspirin, folate and B12 and my homocystine levels are perfect. My doctor only wants me on aspirin but everything I read suggests lovenox and herparin are the standard treatment for homo MTHFR in the US and the UK.

Also, is there an increased miscarriage risk due to us both being homo MTHFR? I understand our bub will be hetero MTHFR but that is all I believe the risk to be.

    Thanks for your help

Hi My understanding would be consistent with yours. Dr james Morton is a leading haematologist and has recently seen one of my patients and told her that Clexane is the treatment of choice not Aspirin. Re your other question, perhaps consulting a geneticist is the way forward there. Good Luck GS
