View Full Version : hindwater leak? rupture of membranes?

Sometimes it's hard to know if you've had a complete rupture or a hindwater leak. Especially for first time mamas who have no comparison ;) This might help you sort out whether you've had a hindwater leak or a full ROM, and what you'd like to do about it - if anything! Of course staying home to birth things are a little different but it's still useful to know in case you're a mama who has ROM and no labour for a while. It's all normal!

A hindwater leak occurs when only a small amount of fluid is released. There can be many reasons for this occurring rather than a full ROM.

Obviously, the risk of infection will be higher when there is a large opening in the amniotic sac directly over the cervix, and the risk of infection will be almost negligible when the fluid is leaking from a high leak in the hindwaters. Bacteria would have to be awfully clever to find their way against a tide of outflowing fluid to find a small opening high in the amniotic sac.

Some ways to tell that the fluid is coming from a high leak in the hindwaters:

Belly still feels full of fluid, and baby's body isn't clearly outlined in the belly
Without touching the cervix, do a vaginal exam with sterile gloves to palpate the lower uterine segment directly inside the vagina. If the baby is easily ballotable, meaning it kind of floats up a bit, then there's still fluid in the belly, and the leak isn't over the cervix.

There are two layers of membranes - the outer sac (the chorion), and the inner sac (the amnion); it is possible for the outer sac to break while the inner sac remains intact; this can still release some fluid that might have accumulated between the layers. This would tend to be a small amount of fluid, and generally there is not a continuing trickle.
It is possible for baby to poke a hole through the membranes at some point. Then, as fluid is released, the sac may double over on itself at that point and, like Glad Wrap, stick to itself, forming a seal over the leak. Again, this would tend to be a small amount of fluid, and generally there is not a continuing trickle.

There is no inherent benefit either way. Some women find drippy labours irritating after ROM, some women’s membranes do not rupture until crowning, some never do and the baby is born “in the caul”. If you’re birthing in a hospital, regardless of how much fluid has left the building, you can still stay home until contractions are strong and close together or you’ll be on a timetable and possibly pressured to induce/augment. You will be told that you are at risk of infection but it is very slim indeed provided you follow normal hygienic practices for toileting and avoid sex.

NICE guidelines from the UK state that women can go up to 96 hours without being induced. The likelihood of infection is significantly reduced if you put nothing in your vagina (this includes VEs which are a major way germs are spread) and stay home in your own friendly germ environment as long as possible.

5.5 Induction of Labour in the presence of Prelabour Rupture of the Membranes (page 2

5.5.1 ....Epidemiological data on time interval from term PROM to spontaneous labour demonstrates that most women go into spontaneous labour within 24 hours of rupturing their membranes.

86% of women will labour within 12-23 hours
91% will labour within 24-47 hours
94% will labour within 48-95 hours
6% of women will not be in spontaneous labour witthin 96 hours of PROM.

hi janet, thanks for that!

when i was 28 wks with Tilly i was still in hospital recovering from a stint in intensive care for a severe asthma attack. anyway, one morning i felt a huge gush of fluid and it was confirmed that my waters had broken and i was sent to RPA where i had to spend the next 4 wks, as my local hospital could only take babies from 32 wks, and they had to keep a constant watch for infection etc. i continued to leak fluid for about 3 days after, then nothing the whole time i was there, and further ultrasounds showed an almost normal level of amniotic fluid. they said it was probably a hindwater leak, and that it is always to be safe than sorry, true enough, but i remember thinking 'surely there has to be a way of determining what sort of leak it is??' being away from dh and children during that time was the hardest thing i have ever ever had to do, and it would have been great if it could have been avoided.

Oh wow! How scary! Being away from your bubs must have been awful :( And it sounds like it sealed up again beautifully which many leaks do, even during labour. How clever are our bodies, hey?!

With my second child my waters leaked at around 24 weeks. I was on bedrest for 2 weeks in hospital, but got a day leave to attend my own wedding

When I was upright again, my baby slid down and her bead blocked the hole!!! She was born on her due date :)

With my 4th, I thought I was peeing my pants occasioanly for the last 4 days. I had an ultrasound which showed very low fluid, so I was induced. My hospital records say I was induced for prolonged hind waters. Weird thing is, when the doctor came in to break my waters properly, there was a massive gush  so much for low fluid huh!

Hey guys :)

heck- I was worried about whether I would KNOW if my waters broke - but there wasn't really much doubt!!! I stood in the shower (fully clothed - due to unexpectedness of the situation... I have never run so fast from the carpeted bedroom before!! ) and it just GUSHED out... I dont know HOW there was room for Jack in there!!

it continued to gush EVERY contraction from then on - which lead to much stress in the car on the way to the hospital (damn those red lights!! ) so much for the advice of 'put a pad on' - HA! I went through 4 maternity pads before GETTING in the car!!!

I guess Jack was determined to make an entrance!! :D

I agree Apprenticmomma!I thought it was cool.

Getting back onto leaks...........My waters broke 16 days before I had my bubba (induced). It was major gush and nothing followed.Plus the midwives at my local hosp. wouldn't believe they had broken until she was born with NO liquor. I'm still very upset about this, because I knew they had broken and they treated me like I was a child who didn't know anything. "Oh you must have just wee'd yourself"
Bubs is only 6 days old today so my labour, and preceeding problems are still fresh (ha ha not to mention my hormone levels ;) ).I just hope they send me a survey form!! My answers will not be nice, especially since they refused point blank to give me an ultrasound to check my liquid levels.

My hindwaters broke the morning my baby boy was born. I stood in a pool of liquid, went to the bathroom - more pooling, back outside (you get the picture). I rang the hospital and the nurses told me also that I must have peed myself so no need to come in. When I finally got hold of my OB, he told me to go in straight away.

Charming - I don't think you could easily confuse the two ! :D

hehe yes, it would save us all alot of time :)

nara and rynosmum, stories like this make me soooo mad, what sort of stupid woman/man, would tell a pregnant woman that her waters couldnt have broken, without even checking???? some people are in the wrong profession i think......

So true Coopsntilly, some people are definitely in the wrong profession! Including the midwife who told me I would never breastfeed because 1.my baby was so small and 2. because I have crappy nipples........."so you may as well go straight to the chemist and buy bottles and formula"

Guess what you old hag, I have been exclusively breastfeeding for the last 5 days and we're getting better and better each time!!(I had been comping with EBM)

I seemed to cop the worst of the midwives throughout pregnancy and labour and 2 days following!Hmm they wanted me to stay for an extra night!!Thanks but no thanks!

That is exactly what happened to me! I thought that my waters had broken as there was loads of fluid. I was asked to come into hospital and failed to go into labour so was induced 12 hours later. I was on the drip for 12hours and still failed to go into labour. After examination it was discovered that my forewaters were still intact and that only my hindwaters had ruptured. OB then ruptured membranes. I went into fullblown labour with contractions coming on top on eachother as the Midwives rushed to turn down the induction drip. 13 hours later bubba emerged - after gas, peth, epidural and vacume. I was so absolutely traumatised and so angry about what had happened. I kept getting nightmares for the weeks following the birth. I really feel as though my bubby was not ready to come and that the hospital just rushed us. However she is the most beautiful thing on the planet and it was all worth it!!!

I'm sorry that happened to you, Kylie. I wish we could make sure that we separate the baby from the birth in this society. You had a really terrible birth experience but it doesn't mean that you don't adore your baby. I hope you get lots of support to recover. I suppose you won't be going to the hospy so soon next time! Healing vibes to you ~*~*~*~*~*~*~*

I woke up on the 14th with a trickle which continued for over an hour. I went to hospital and they said that I had had a hindwater leak.... da** it, no baby and I was DYING to get her out!

Then I woke up with the same thing on the 18th, and the hospital said that I had to stay in hospital, so could either be induced or wait to go into labour naturally, and be induced after 48 hours if that didnt work. I figured, well induce me then, I want this baby out NOW!!!!! So they did and I had a four hours 11 labour with a posterior baby and an epidural.

I have had the same "hindwater leak" this time and went to hospital only to be told by the doc that there is no such thing as a hindwater leak, your waters either break or they dont and I must have wet myself. I felt like such a child and like I was being told off for something.

I have since got my own ob just in case I ever feel like someone is telling me to shutup and sit in the corner again!!!
