The only thing I might suggest would be to add the year to the date - thats done sometimes on formal invites, nice for keepsakes etc.

Will you have a specific dress code? Some guests may want to know. Perhaps you can include this information on a seperate "Guests Information" Page and you can put in Dress Code, any guest accommodation info (for out of towners), maps and wishing well poem.

hmm no dress code.. ceremony is in a park & reception at a function center.. so its low key..

should i add my number to rsvp? or just put address/phone number on back of envelope?

Add phone number to invite, we didn't do this and a lot of people threw out the envelope so they didn't know how to get in touch with us.
