Child Constipated

I have a 4 year old who for the past 2 years had been holding her poo not wanting to go to the toilet. We have been to numerous doctors, have tried all sorts of laxatives. But once she feels it coming she hides and holds it to the poit where it is very painful for her. Has anyone had this problem with their child? Any useful tips please?
