- Post your form, along with a recent photo of yourself and a maximum 300-word story on why you'd like a makeover, to PO Box 216, Camperdown NSW 1450.
- Competition closes July 1 2009.
- You must be available for a photo shoot on Saturday, July 11 2009.
- The winner will be announced in My Child's Spring issue, on Sale August 19.
